Breast Lift Without Implants: Who It Suits and Results
By Alluring Editorial TeamPublished
Can you get a breast lift without implants?
Yes. A lift alone raises and reshapes your own tissue by removing stretched skin and moving the nipple higher. The American Society of Plastic Surgeons says it doesn't significantly change your size or round out the upper breast. Cleveland Clinic says results should last for several years, though aging, pregnancy and weight changes still affect them.

Can you get a breast lift without implants?
Yes. A breast lift doesn't need implants: the surgery removes stretched skin, tightens the tissue around it and moves the nipple and areola higher, all using your own tissue. Implants are an add-on for women who also want more fullness. Our page explains what a breast lift involves.
What can a breast lift without implants do, and what can't it do?
A lift alone changes the position and shape of your breasts, not their size. The American Society of Plastic Surgeons (ASPS) says it "does not significantly change the size of your breasts or round out the upper part of your breast." Cleveland Clinic puts it more simply: "A breast lift alone won't change the size of your breasts."
| A lift alone can | A lift alone can't |
|---|---|
| Raise the breast and move the nipple and areola higher | Add size |
| Remove stretched skin and tighten the tissue | Round out the upper part of the breast |
| Reduce an areola that has stretched | Make breasts noticeably smaller |
| Turn an areola that points downward to face forward | Avoid scars, which are permanent but fade |
| Make your breasts more even | Stop aging and gravity from changing your breasts |
Who is a breast lift without implants for?
It suits you if you like your size but not how your breasts sit. The Aesthetic Society says: "If you are happy with your breast size, but not the way your breasts look, you may benefit from a breast lift." ASPS's candidate list adds:
- You are physically healthy and maintain a stable weight.
- You do not smoke.
- When unsupported, your nipples fall below the breast crease.
- Your nipples and areolas point downward.
- You have stretched skin and enlarged areolas.
- One breast is lower than the other.
If you plan to have children, The Aesthetic Society says you may want to postpone, because pregnancy may stretch the breasts and reduce their volume. Your surgeon decides at consultation. Until then, you can check where your breasts sit at home and read the signs surgeons look for.
How do I know if I need implants too?
You may want volume too if, after picturing your breasts higher, you would still like them fuller. ASPS says: "If you want your breasts to look fuller, consider breast lift and breast augmentation." Three signs point that way:
- You want a fuller upper breast. A lift alone doesn't round it out, while ASPS says an implant "can provide improved upper breast fullness or cleavage."
- You have lost volume. Lawrence Iteld, MD, writing on the ASPS website, names pregnancy, breastfeeding, weight loss and aging as causes.
- You want to go up in size. Cleveland Clinic says to consider augmentation for that.
A rough way to picture it at home: in a mirror, gently raise your breasts with your hands to where you would like them to sit. If you like what you see, a lift alone may match your goal. If you still wish for more fullness, ask about a breast lift with implants. This isn't a test from any study, only a way to frame the question for your consultation.
What do results look like after a lift without implants?
Your breasts look higher right away, and the final shape appears over the next few months, ASPS says. Many people also report that their bras and clothes fit better, Cleveland Clinic says. Your surgeon sets your own schedule, but the published timeline looks like this:
| When | What to expect | Source |
|---|---|---|
| Right away | Results are visible | ASPS |
| 2 to 5 days | Your chest may feel stiff and sore | The Aesthetic Society |
| About 1 week | Many women go back to desk work and gentle activities such as walking | Cleveland Clinic |
| First 2 weeks | No lifting, driving, laundry or cleaning | The Aesthetic Society |
| About 2 weeks | Discomfort, swelling and bruising ease | Cleveland Clinic |
| Up to 6 weeks | Lifting, exercise and sex may not be cleared until then | Cleveland Clinic |
| About 8 weeks | Women can usually wear a regular bra | Cleveland Clinic |
| The next few months | Shape and position settle into the final result | ASPS |
| Many months | Incisions stay red or pink | The Aesthetic Society |
| About 1 year | Most of the scar fading has happened | Cleveland Clinic's surgery center |

Call 911 or go to the emergency room for new or severe chest pain, trouble breathing, or pain, redness or swelling in one leg, which MedlinePlus says can be signs of a blood clot in the lung. Call your surgical team immediately if either breast becomes red, hot, swollen or painful, or if you have bleeding, drainage, redness or swelling at an incision, dizziness, a fever higher than 100.5°F, severe pain not helped with medicine, or nausea or vomiting. That list is Cleveland Clinic's.

How long does a breast lift without implants last?
It is long-lasting, but the sources we read don't agree on how long. ASPS says results "will be long-lasting" while your breasts keep changing with aging and gravity, and that you can keep your new look longer if you maintain your weight and keep a healthy lifestyle. Cleveland Clinic says results "should last for several years." The Aesthetic Society says results "might slowly diminish over time because of gravity and aging."
Pregnancy and significant weight loss can undo some of the change, ASPS says. Our post on when to have a lift after pregnancy covers this in more depth.
What happens to fullness after a lift?
A lift moves your existing tissue rather than adding to it, so expect a better position and shape, not more volume. Cleveland Clinic says a lift can reshape the breast "to a more rounded appearance," but ASPS says it won't round out the upper part. Cleveland Clinic's surgery center notes that lifts are often combined with augmentation "to add volume to the upper portion of the breast."
We found no source that gives a figure for how much bra size changes after a lift, so ask your surgeon what to expect for you.
Fat transfer is another way to add a little volume: ASPS describes fat transfer breast augmentation as moving fat from elsewhere on your body by liposuction, for women looking for "a relatively small increase in breast size."
What are the risks of a breast lift without implants?
Significant complications are infrequent, The Aesthetic Society says, but this is still surgery. Two studies give a sense of the numbers:
- The first 30 days. In a national surgical database, complications followed 2.5% of breast lifts alone, and reoperations 1.4% (Ali and colleagues, 2021).
- A wider review. A 2019 review of 34 studies and 1,888 patients who had a lift without implants found complications in 10.4%, most often scar problems (3%) and nipple and areola problems such as asymmetry or reduced sensation (2.9%) (di Summa and colleagues, 2019). Its studies span 1980 to 2016, with mixed techniques.
Feeling gradually returns over weeks or months, numbness can last up to 1 year for some, and loss of feeling is rarely permanent, Cleveland Clinic's surgery center says. Cleveland Clinic lists inability to breastfeed among the risks, and The Aesthetic Society lists difficulty breastfeeding. Shahram Salemy, MD, writing on the ASPS website, says many women have breastfed successfully afterward, so raise it if you plan to nurse. Our page covers breast lift risks and safety.

Frequently asked questions
Can you get a breast lift if you have small breasts?
Yes. Cleveland Clinic's surgery center says patients with smaller breasts and minimal sagging may be candidates for a procedure with smaller incisions than a standard lift. A lift alone won't make them bigger, so if you have too little volume, The Aesthetic Society says your surgeon may recommend augmentation as well.
Can you get a breast lift if you have large breasts?
Yes, but The Aesthetic Society says results "may not be as long-lasting" as on smaller breasts, because "the weight of larger breasts works against surgical changes." If you would also like smaller breasts, ASPS suggests combining a lift with a reduction. Our guide to a breast lift vs a reduction can help you choose.
Does a breast lift without implants change your size?
Not significantly, ASPS says. Shahram Salemy, MD, writing on the ASPS website, says a lift "is primarily designed to lift and reshape the breasts, not to change their size." Your breasts may look different in clothes because they sit higher, and surgeons remove skin, so ask yours how your bra size may change.
How is a breast lift done without implants?
Your surgeon makes incisions, removes extra skin, reshapes the breast and moves the nipple and areola higher, shrinking the areola if needed, ASPS says. Cleveland Clinic's health library says it usually takes 1–2 hours, while its surgery center says two and a half to three, so ask your surgeon how long yours will take. Our guide explains the types of breast lift.
Can you add implants later?
Yes, it's possible. ASPS says implants "can be placed at the same time or as a second procedure, depending on the experience of the surgeon." A later implant means another surgery and another recovery, so if you are unsure, raise it before your lift so your surgeon can plan with that in mind.
Is a lift alone safer than a lift with implants?
The studies point different ways. In one national database, complications within 30 days followed 2.5% of lifts alone and 1.5% of lifts with implants, with reoperations of 1.4% and 1.5% (Ali and colleagues, 2021). In one practice's review, 23.2% had a reoperation after a lift with implants and 10.2% after a lift alone (Calobrace and colleagues, 2013). The FDA also says implants need monitoring and to assume more surgery later.
Can a breast lift fix uneven breasts?
It can help. One breast sitting lower than the other is on ASPS's candidate list, and Cleveland Clinic says a lift can make your breasts more symmetrical. Don't expect a perfect match, though: ASPS lists breast asymmetry among the risks. Your surgeon can tell you at your exam how much evenness to expect.
Can you get a breast lift without scars?
Not with surgery. The Aesthetic Society says that even minimal-scar options "all require incisions of some sort," and give less lift. ASPS says incision lines are permanent but in most cases fade. Our posts cover where the scars sit and what non-surgical treatments can and can't do.
