Plan on about two weeks off work, eight weeks of protecting your buttocks and three to six months to see your final shape. Don't sit directly on your buttocks for at least two weeks, then sit only briefly on a BBL pillow until about week 8. Wear your compression garment around the clock for the first month. Most people feel recovered at two to three months.
How long does BBL recovery take?
Most people go back to a desk job 10 to 14 days after a Brazilian butt lift (BBL), sit normally and exercise again at about 8 weeks, and see their final shape 3 to 6 months after surgery. Cleveland Clinic puts full recovery at two to three months and notes that it can take up to six.
BBL recovery takes this long because grafted fat is fragile. Pressure on the buttocks in the first weeks puts the transferred fat at risk, which is why most BBL recovery rules are about how you sit, sleep and move.

Your surgeon's instructions come first. The ranges above come from Cleveland Clinic, The Aesthetic Society and plastic surgeons interviewed by the American Society of Plastic Surgeons (ASPS). Your own BBL recovery plan also depends on how many areas were liposuctioned and what else was done in the same surgery.
What happens in the first two weeks after a BBL?
The first two weeks are the strictest part of BBL recovery: no sitting or lying on your buttocks, a compression garment around the clock and short walks to keep your blood moving. Cleveland Clinic says you won't be able to sit or lie on your buttocks for at least two weeks, so you stand, walk or lie on your stomach or side instead.
Arrange help at home before surgery. The Aesthetic Society notes that most people don't feel like themselves for one or two weeks and shouldn't do anything strenuous, including lifting, in that time.
How much does a BBL hurt in the first week?
The first several days after a BBL are usually the most painful, and most people need some pain medication for the first four to five days, according to a plastic surgeon interviewed by ASPS. The areas where fat was removed tend to feel tender and bruised. Cleveland Clinic says the pain should ease after one to two weeks.
Short walks start early even so. The Aesthetic Society says you should be able to get up and walk after the second day, which helps prevent blood clots.
When can you sit, and how should you sleep?
You can't sit directly on your buttocks for at least two weeks after a BBL. After that, surgeons allow short periods on a BBL pillow, a triangle-shaped cushion that shifts your weight onto your thighs. One surgeon interviewed by ASPS keeps sitting to about 10 minutes at a time for the first six weeks, and Cleveland Clinic says most people can't sit without the pillow for about eight weeks. Sleep on your stomach or side, as Cleveland Clinic advises, and stay off your back until your surgeon clears it. What should you expect at 2, 4, 6 and 8 weeks after a BBL?
BBL recovery loosens in steps: desk work around two weeks, a lighter garment schedule and fast walks after the first month, and normal sitting and exercise around eight weeks.
2 weeks post-op
Two weeks after a BBL, most people are back at a desk job or close to it, and the worst pain is behind them. The Aesthetic Society says you can return to work, drive or go to the movies after 10 to 14 days, as long as you sit on a pillow so there's no pressure on your buttocks. You're still wearing the compression garment 24/7 and still avoiding heavy lifting.
4 weeks post-op
Four weeks (one month) after a BBL, the round-the-clock garment phase ends for many patients, and Cleveland Clinic's one-month limit on heavy lifting and strenuous activity is up; ask your surgeon before restarting either. You're still sitting only briefly, on a pillow. Swelling is going down and some of the grafted fat is reabsorbed, so your buttocks usually look smaller than they did in week one. That's expected, and it's too early to judge the result.
5 and 6 weeks post-op
Five and six weeks after a BBL, you're in the second month of compression, wearing the garment at least 12 hours a day (many patients do it overnight), and sitting on a BBL pillow in short stretches. Light activity such as fast-paced walking is fine after the first month, according to an ASPS article by plastic surgeon Kamran Azad, MD, but workouts that load your buttocks still wait. Swelling keeps easing and your shape keeps changing for months.
8 weeks post-op
Around eight weeks after a BBL, most day-to-day limits lift. Cleveland Clinic says you won't be able to sit without your pillow for approximately eight weeks, and The Aesthetic Society says normal activities and exercise can resume after eight weeks. Surgeons differ on how hard to start: Cleveland Clinic describes light exercise at this point, so get cleared before you go back to squats, running or heavy weights.
How long do you wear a compression garment after a BBL?
Wear your compression garment 24/7, except in the shower, for the first month after a BBL, then at least 12 hours a day in the second month, or as your surgeon directs. That schedule comes from plastic surgeon Dr. Funderburk in the ASPS recovery article; Dr. Azad's ASPS article recommends eight weeks of wear. The evidence behind any exact duration is thin: a 2023 review in Plastic and Reconstructive Surgery Global Open found little high-quality research on how long compression is needed, so your surgeon's schedule is the one to follow.
Two practical points matter in Miami's heat and humidity. Keep two garments so one is always clean and dry, and swap them when one gets damp; taking the garment off briefly to change it is fine. When can you go back to work, drive and exercise after a BBL?
Most people return to desk work and driving 10 to 14 days after a BBL, sitting on a pillow, according to Cleveland Clinic and The Aesthetic Society. Some surgeons allow work sooner: plastic surgeons interviewed by ASPS say their patients go back between days 7 and 10. A job that involves lifting needs a longer break, so ask your surgeon for a return date before you book time off.
Exercise after a BBL comes back in stages:
- From day 1 or 2: short walks. The Aesthetic Society says walking after the second day helps prevent blood clots.
- First month: no heavy lifting or strenuous activity, per Cleveland Clinic.
- After a month: light activity such as fast-paced walking.
- Around 8 weeks: normal exercise, once your surgeon clears you. Ask specifically about squats, lunges and running before you restart them.

Which BBL recovery mistakes cost you grafted fat?
The BBL recovery mistakes that cost the most fat put pressure on your buttocks too early. Most surgeons tell patients to expect to lose about 30% of the transferred fat even with good aftercare, according to an ASPS article on traveling after a BBL, so the aim is to avoid losing more.
- Sitting on your buttocks in the first two weeks. Surgeons interviewed by ASPS say sitting too soon puts you at risk of uneven or quickly shrinking results. If it happens by accident, tell your surgeon.
- Sitting too long, even on a pillow. Keep to short stretches, about 10 minutes at a time for the first six weeks by one ASPS surgeon's rule, and stand up between them.
- Sleeping on your back before you're cleared. Lying on your back is pressure on the graft, just like sitting.
- Skipping or loosening the garment in the first month.
- Staying in bed. Walking from the first days keeps blood moving and lowers the risk of clots.
- Going back to the gym early. No heavy lifting or strenuous activity for the first month, and no normal workouts until you're cleared at around eight weeks.
- Smoking or vaping. The Aesthetic Society says not smoking improves healing and helps prevent complications, and Cleveland Clinic lists smoking, including e-cigarettes, as a risk factor for surgical wound infection.
- Judging the result too early. The Aesthetic Society advises waiting at least three months before shopping for new clothes, because swelling hides the final shape.
When is it safe to fly home after a BBL?
Plan to stay near your surgeon for at least four to five days after a BBL, and longer if you can. ASPS's article on traveling after a BBL explains why: an infection typically shows up three to five days after surgery, and if it does, your surgeon needs to see you in person. If you're traveling to Miami from another state, Alluring recommends staying 7 to 10 days, so you can be seen at follow-up and cleared before you fly. Sitting for long periods during travel raises the risk of blood clots, and the risk rises the longer you fly. On the way home:
- Get up and walk for 5 to 10 minutes for every hour you sit, the advice a plastic surgeon gives in the ASPS travel article.
- Drink plenty of water.
- Wear compression stockings if your surgeon recommends them.
- Plan on sitting upright on your BBL pillow. Airlines won't let you lie across seats for the flight.
Clots can form days or even weeks after a flight, so know the warning signs in the next section before you travel.
How safe is a BBL in 2026?
A BBL is safest when the fat stays under the skin, above the gluteal muscle, and in Florida that is now the law. The main danger is a fat embolism: Cleveland Clinic explains that the buttocks contain many blood vessels, and if the cannula strikes one, fat can enter the bloodstream, which can be fatal.
- Florida's rules. Under Florida Statutes §458.328, fat may only be injected into the space under the skin and may not cross the fascia over the gluteal muscle. The surgeon must use ultrasound guidance while moving the cannula, stay with one patient for the whole procedure, examine you in person no later than the day before surgery, and do the fat removal and injection personally.
- Why the rules exist. South Florida recorded 25 deaths from BBL fat embolism between 2010 and 2022, 14 of them after Florida's 2019 under-the-skin-only rule, according to a 2023 study by plastic surgeons Pazmiño and Garcia in Aesthetic Surgery Journal. 92% of those patients had surgery at high-volume, budget clinics, and in every case examined at autopsy, fat had been injected into the muscle.
- What surgeon surveys show. Surveys estimated the BBL death rate at 1 in 3,448 in 2017 and 1 in 14,952 in 2019, after surgical safety guidance changed (Rios and Gupta, Aesthetic Surgery Journal, 2020). Those figures are self-reported, and South Florida's deaths continued after 2019, so read them as a trend rather than today's odds.
- Complications overall. A 2026 meta-analysis of 38 studies and 22,151 patients found minor complications in 3.58% of BBL patients, most often a seroma (a pocket of fluid) in 2.03%, and pulmonary embolism in 0.04%. Major complications were less common with ultrasound guidance: 0.02% versus 0.08% (Elsaftawy and colleagues, Plastic and Reconstructive Surgery, 2026).
- How common BBLs are. ASPS estimates that 25,662 BBLs were performed in the US in 2025, 5% fewer than in 2024, according to its 2025 Plastic Surgery Statistics Report, released September 1, 2026.
Ask any surgeon you consider, including us, where they place the fat, whether they use ultrasound and how many BBLs they run at the same time. A clinic that can't answer clearly is a clinic to walk away from.
Which symptoms mean you should call your surgeon now?
Call 911 for difficulty breathing, a faster-than-normal heartbeat, chest pain that gets worse when you cough or breathe deeply, coughing up blood, or lightheadedness or fainting. After a BBL these can be signs of a blood clot or fat in the lungs, and they need emergency care, not a callback.
Call your surgeon the same day for:
- Signs of infection, which typically appear three to seven days after surgery: a noticeable odor from an incision, pus, redness, warmth, pain when you touch the area, or a fever above 101°F (38.4°C), according to Cleveland Clinic. - A swollen, painful leg or skin that is red and warm to the touch, especially after travel, which can mean a blood clot.
- A soft, swollen pocket that feels like it holds fluid, which can be a seroma.
- A firm lump in the buttocks that hurts or grows. Fat necrosis, where some grafted fat dies and hardens, usually shows up as a painless firm lump or oil cyst, but a lump that hurts, grows or drains needs to be checked.
- Pain that gets worse instead of better after the first week.
When will you see your final BBL results?
You'll see your final BBL shape 3 to 6 months after surgery, once swelling has settled. The acute risk of losing fat from the buttocks subsides around month three, according to the ASPS recovery article, and during months three to six the shape refines and the curve becomes more apparent.
About 50% to 80% of grafted fat survives a BBL. Plastic surgeons interviewed by ASPS put the average "take" around 60%, and a 2020 review in Seminars in Plastic Surgery estimates that 20% to 50% of grafted fat is reabsorbed.
