Do BBLs Stink? What BBL Smell Is and How Long It Lasts

What is BBL smell?

A Brazilian butt lift (BBL) that has healed doesn't smell. What people online call "BBL smell" is odor during recovery, and it has ordinary causes: fluid draining from the small liposuction incisions, and sweat held against the skin by a compression garment worn around the clock. Both fade as the incisions stop draining and close.

The American Society of Plastic Surgeons (ASPS) says BBL smell "is not a common or expected outcome of a BBL when performed under sterile conditions with proper postoperative care," and notes that the topic took off through social media posts that "can sometimes exaggerate or misattribute postoperative experiences." A smell is worth taking seriously in one situation: when it comes from an incision along with pus, spreading redness or a fever, because that can be an infection.

What causes BBL smell after surgery?

Most BBL odor comes from moisture: drainage, sweat and a garment that stays damp. Less often it is a sign of a complication.

• Drainage from the liposuction incisions. During liposuction, surgeons inject fluid to loosen the fat, and some of it seeps back out afterward, mixed with a little blood. The Aesthetic Society says incisions "may be leaky or drain fluids for several days." When that fluid soaks into pads or a garment and sits there, it starts to smell.
• A garment that traps heat and sweat. Compression is worn day and night in the first month. A surgeon interviewed by ASPS names bacterial overgrowth on the skin, "due to moisture, heat and friction, especially if patients wear the same garment for long periods without cleaning it," as a main cause.
• Hard-to-reach hygiene. You can't sit on your buttocks for at least two weeks, and bending is uncomfortable, so cleaning up after using the bathroom is harder than usual. ASPS lists poor hygiene, "such as not wiping well," due to limited mobility, as another cause.
• Infection. A noticeable odor from an incision is one of the signs of a surgical wound infection, along with thick, cloudy discharge, redness and fever, according to Cleveland Clinic.
• An infected seroma. A seroma is a pocket of fluid under the skin. Cleveland Clinic explains that it can become infected, and then the fluid can turn pus-like and smell.
• Fat necrosis, rarely. When some grafted fat doesn't survive, it can harden into a firm lump or an oil cyst. The ASPS surgeon says odor is rarely a sign of early infection or fat necrosis, "though that usually presents with additional symptoms like redness, fever, increasing pain and/or a firm, tender nodule developing."

Is BBL odor normal, or a sign of infection?

The difference is rarely the smell alone. Look at where the odor comes from, whether it is fading or getting stronger, and what comes with it.

The signs in the right-hand column come from Cleveland Clinic's guides to surgical wound infection, seroma and fat necrosis. Cleveland Clinic says infection symptoms typically show up three to seven days after surgery, so a new or stronger smell in that first week deserves a call rather than more deodorant.

How long does BBL smell last?

Normal BBL odor lasts about as long as the incisions drain and the garment stays damp. The Aesthetic Society says incisions can leak for several days, and the smell usually fades after that, as long as the garment is washed and your skin is kept clean and dry.

There is no set number of days after which a smell becomes dangerous. What matters is the direction: a smell that is fading is healing, and a smell that is getting stronger, or one that appears a week or more after surgery with other symptoms, is not. If the odor stays even though you shower as directed and change into a clean garment every day, ask your surgeon to look at the incisions.

Do BBLs stink years later?

No. Once the incisions have closed and you no longer wear a compression garment, a BBL has nothing that would produce an odor, so a smell months or years after surgery has a different cause. Body odor, sweat and skin conditions aren't changed by a BBL.

The one late BBL issue to watch for is a lump. Fat necrosis usually appears as a firm lump or oil cyst and is often painless, according to Cleveland Clinic. See a doctor if a lump where fat was grafted hurts, grows, turns red or drains.

What does BBL smell like?

People usually describe BBL odor as the smell of a sweaty, damp garment: sour or musty. It tends to be strongest when you take the garment off, and it goes away when the garment is washed and your skin is clean.

A smell like stool usually means the garment got soiled, which is easy to miss when you can't twist or sit to check. Change into a clean garment and wash the soiled one right away. A strong, foul smell from an incision itself, especially with thick, cloudy discharge, is different. That is the smell Cleveland Clinic lists among the signs of infection.

How to prevent BBL smell during recovery

Most BBL odor is preventable with a routine that keeps the garment clean and your skin dry. Follow your surgeon's written aftercare instructions first; these steps fit inside them.

1. Buy two compression garments before surgery. The ASPS surgeon advises changing and washing garments "regularly, ideally every 24 hours." With two, one is always clean and dry, and taking the garment off briefly to change it is fine.
2. Line the garment. Clean, breathable dressings or liners inside the garment "can help absorb moisture and reduce bacterial buildup," according to the same ASPS article. Change them whenever they are damp, more often while the incisions are draining.
3. Shower when your surgeon allows it, and ask whether to use an antiseptic wash. The ASPS surgeon has patients use a chlorhexidine cleanser (Hibiclens) before and after surgery. Pat incisions dry instead of rubbing.
4. Make the bathroom easier. Keep flushable wipes or a handheld bidet within reach so you can clean well without sitting or twisting, and change your garment if it gets soiled.
5. Stay cool. Loose, breathable clothes over the garment and air conditioning mean less sweat. That matters more in Miami's heat and humidity.
6. Ask about lymphatic drainage massage. ASPS says professionally performed lymphatic drainage can reduce fluid retention and swelling, "which may indirectly help minimize odor." Our guide to how many massages you need after a BBL covers timing.
7. Don't smoke or vape. Cleveland Clinic lists smoking as a risk factor for surgical wound infection.
8. Keep your follow-up appointments, so your surgeon checks the incisions in person rather than you guessing at home.

Can the way a BBL is done lower the odds of odor?

Partly. The surgeon ASPS interviewed says that when less fat has to be moved, less fluid is injected during surgery, which "usually results in less postoperative fluid drainage, therefore less opportunity for bacterial overgrowth or garment saturation." ASPS also notes that fat transfer is a sterile procedure.

The rest comes down to aftercare and access to your surgeon.
